Information you choose to share

The inquiry form collects your name, email address, selected interests, questionnaire answers, optional message, and consent choices. Please do not submit medical records, diagnoses, medication details, financial information, or other sensitive personal information.

How information is used

Information is used to respond to your request, discuss services, and manage follow-up. If you separately opt in to future email updates, your email and consent may be retained for that purpose. You can withdraw that permission by emailing Lindy. An inquiry does not automatically enroll you in marketing emails.

Services involved in delivery

Forms are processed by FormSubmit and delivered to Lindy’s Gmail inbox. Their privacy and security practices apply to their services. FormSubmit may use a verification or anti-spam step. The site’s hosting provider may process technical information such as IP addresses, request details, and operational logs. Google Fonts is used to display typefaces and receives the browser requests needed to load those fonts.

Website tracking and storage

This website does not add advertising trackers or analytics scripts. Questionnaire selections stay on the page until you submit them; they are not saved to browser storage. External services may use their own cookies or technical storage when you visit or interact with them.

Retention and your choices

Inquiry information is retained as needed to respond, maintain relevant business records, and meet applicable obligations. You may request access, correction, or deletion by emailing Lindy. Requests will be addressed subject to applicable requirements and legitimate recordkeeping needs.

Third-party links

Links to EllieMD and other websites take you to separate services with their own terms and privacy policies. LIFTed with Lindy does not receive your medical records simply because you follow an affiliate link.

Security and children

No website or email system can guarantee absolute security. These forms are intended for adults 18 and older and are not a secure medical intake channel. If you believe a child has submitted personal information, please contact Lindy.
